To make it simple and convenient, Dr. Thomas has broken down the process of getting started into 5 easy steps:

Step 1: Read this website and write your questions for Dr. Thomas on a sheet of paper.

Step 2: Download and print our Medical History Form and Registration Form. Complete forms and fax to Dr. Thomas at 407-956-4923, or mail to Dr. Daniel Thomas at: 212 W. Bay Avenue, Longwood, FL 32750.

Step 3: Call our office at 407-265-1888 to schedule an appointment for a consultation with Dr. Thomas. During this time he will review your medical history and will answer your questions (including complete information on costs). This takes approximately 15 minutes and there is no charge.

Step 4: Dr. Thomas will order the needed lab work to check your current hormone levels. After the lab faxes us your results (usually in 3-5 days), Dr. Thomas will call you to review the results, and will answer any remaining questions you may have. An appointment will then be made for a physical examination and to insert your hormone pellets (same visit).

Step 5: Read a copy of our Informed Consent (Female or Male), and come to the office to have Dr. Thomas perform a physical examination and insert your pellets.
